I have nothing to add to this other than Isla Duala is still one of my top 3 ARMA maps of all time.

@JD_Wang has a great script that allows the hostility of the civ to decrease when in the "detention area". I can't find it off hand but maybe he can repost it.
YES, no more backpack bug with ORBAT. It's very easy to create civs as well. Hit me up on Steam @Opendome if you need me to walk you through it.
I recommend running VCOM in script form on the server. Dominic, the creator of VCOM, has mentioned to me several times that VCOM adds nothing if it is run client side so as long as the server has it...you're good to go.
It's also very easy to adjust settings in the default settings.sqf within the mission folder AND it cuts down on mod dependencies as well.
@Opendome I put together a VCOM specific OPFOR pack as well that takes advantage of the static weapon placement and explosive placement features of VCOM. It's in my Steam Workshop (AuburnAlumni on Steam)
I've done this before and yes..you can. Enterable buildings will be detected by ALiVE and populated. If it's an especially large town, etc. you may want to place small radius Custom Objectives on either end of it so that it's heavily populated as well.
FYI, some of the I44 maps do indeed work.
I have an Iron Front + Faces of War campaign running on my clan's dedi set on I44 Neaville as we speak and its working great.
Yes it affects their hostility towards you.
@Inspire1 Are you asking if the factions are fleshed out? Yes they are. If you want the best of both worlds, you can set IND to a coventional threat and OPFOR as an assymetric threat for example. I have a Steam Workshop campaign that does this called "The Deadly Shadows".
